Senior Health Care Professionals makes available needed behavioral health services to residents of senior care facilities by:
- Providing senior care facilities a system to identify residents in need of behavioral and mental health services and arranging for qualified professionals to diagnose and treat identified needs
- Psychotherapy and Health and Behavioral counseling for the following conditions:
- grief and loss issues
- withdrawal and depression
- disruptive behavioral problems
- adjustment to physical illness
- pain management
- treatment decisions
- Behavioral health solutions for senior care facilities at no cost to the facility and no or minimal cost to facility resident, depending upon individual insurance
- Local clinical coordinator visits contracted facilities on a regular basis to conduct in-service education programs, support quarterly treatment planning and problem solving. Coordinators also manage the interface between facilities and all care professionals to ensure needs are met and expectations fulfilled
- Extensive documentation review programs for completeness, clinical consistency and compliance with HFCA and other third party payor requirements. This includes assisting in integrating behavioral health documentation into individual resident’s nursing care plans
